Acquired Brain Injury (ABI).
Exercise physiology for ABI focuses on functional capacity assessment, graded exercise to manage fatigue, progressive conditioning, and NDIS plan support. James takes a conservative approach with complex ABI presentations and maintains referral pathways for cases requiring multidisciplinary input.
Access note: For clients who cannot use stairs, ground floor access is available at Fitness & Wellbeing Physiology. Please mention this when booking.
What the evidence says.
After an acquired brain injury, whether from trauma, hypoxia, infection or surgery, the brain retains a real capacity to rewire. That capacity, neuroplasticity, is driven by repetition, intensity and task specific practice. Structured exercise is one of the strongest known stimuli for it: aerobic training improves attention, processing speed and mood after ABI, and resistance and balance work rebuild the physical capacity that injury and hospital time strip away.
The research is equally clear about what does not work: rest and waiting. Deconditioning after ABI compounds the injury, because fatigue, low mood and lost confidence feed each other. Graded activity breaks that cycle.
How I treat it.
Every ABI is different, so the program starts with a thorough assessment of what the injury has actually affected: strength, balance, endurance, fatigue patterns, and the everyday tasks that matter to you or the person you support. From there we build a program that respects cognitive load and fatigue. Sessions are structured, predictable and 1:1 in a quiet gym, which matters enormously for clients who find busy environments overwhelming.
I work closely with the rest of the team, neuropsychologists, occupational therapists, speech pathologists and support coordinators, and my reporting is written to slot straight into NDIS plan reviews. Capacity building funding covers this work for most participants.
What we aim for.
- Measurable gains in strength, balance and endurance, retested on a schedule
- Better fatigue management, so good days stop being followed by three bad ones
- Independence in the tasks that matter: stairs, community access, work or study tolerance
- A program the client can eventually run with less support, not more
