These terms cover services provided by Fitness & Wellbeing Physiology (ABN 25 547 158 113), a sole-practitioner clinic run by James Murphy, Accredited Exercise Physiologist and Remedial Massage Therapist, at 40/42 Franklin St, Griffith ACT 2603. By booking an appointment you agree to these terms.
Services
I provide exercise physiology and remedial massage services in accordance with ESSA professional standards and my massage qualifications. My services support, but do not replace, care from your GP or medical specialists. If anything in your presentation needs medical review, imaging or a different clinician, I will tell you and refer you on.
Appointments and cancellations
Appointments can be booked online, by phone on 0418 153 608, or by email. I ask for 24 hours' notice if you need to cancel or reschedule. Late cancellations or missed appointments without notice may incur a fee of up to the full session rate, because as a solo practitioner an unfilled slot usually cannot be re-booked at short notice. Fees for missed funded appointments (NDIS, DVA and similar) follow the rules of the relevant scheme. If you are unwell, please call: I would always rather reschedule you than have you push through.
Fees and payment
Current fees are listed on the fees page and are payable at the time of service unless your funding scheme pays me directly. Medicare CDM, DVA, NDIS and workers compensation clients are billed according to the rules and rates of each scheme. Private health rebates are processed where available. Fees are reviewed from time to time and any change is published on the fees page before it applies to you.
Referrals and funding
Some services require a valid referral before treatment can be funded: a GP Management Plan and Team Care Arrangement for Medicare CDM sessions, a D904 or equivalent referral for DVA, and insurer approval for compensation claims. It is your responsibility to make sure your referral or plan is current. If a funding body declines a claim, for example because a referral has expired or sessions are exhausted, the private rate for that session becomes payable by you. I will always flag it beforehand if I can see a funding problem coming.
Your part in treatment
Exercise-based care works through participation. I will always explain what a program involves and why, and adjust it to your capacity, but outcomes depend on the work happening between sessions as well as in them. Please tell me about any change in your health, medications or symptoms, since safe prescription depends on accurate information.
Communication with your treating team
With your consent I communicate with your GP, specialists and other treating practitioners as your care requires, including the reports that Medicare and DVA referrals oblige me to send. Details are in the privacy policy.
Telehealth
Telehealth consultations are available where clinically appropriate, delivered by secure video link. The same fees, cancellation terms and clinical standards apply. If a technical failure prevents a session from proceeding, it will be rescheduled without charge.
Feedback and complaints
If something is not right, tell me directly at admin@fwphysiology.com or 0418 153 608 and I will work to resolve it quickly. You may also contact ESSA regarding professional standards, or the ACT Health Services Commissioner if you prefer an independent avenue.
Changes to these terms
I may update these terms from time to time. The current version is always published on this page, and material changes affecting existing clients will be communicated before they take effect.
