Multiple Sclerosis (MS).
Exercise in MS is strongly evidence-supported for reducing fatigue, improving strength, mood, cognitive function, and quality of life. James tailors exercise intensity to minimise Uhthoff's phenomenon risk, builds fatigue management strategies into session structure, and adjusts programs during periods of relapse.
Access note: For clients who cannot use stairs, ground floor access is available at Fitness & Wellbeing Physiology. Please mention this when booking.
What the evidence says.
The advice given to people with MS for decades, avoid exertion, was wrong. Modern evidence is unambiguous: exercise training in MS improves strength, walking capacity, balance and fatigue, and may have disease modifying effects on top. Resistance training is safe and effective across most disability levels, and aerobic work improves the fatigue that many people with MS rate as their worst symptom.
How I treat it.
MS is a moving target, so the program moves with it. We establish baselines across strength, walking, balance and heat tolerance, then train progressively while respecting Uhthoff symptoms, cooling strategies, timing sessions around your best hours, and adjusting on relapse. Balance and gait work is woven through everything, because falls prevention in MS pays compound interest.
This work is NDIS fundable for most participants, and I write progress reports that plan reviews can actually use.
What we aim for.
- Walking further, steadier and with less fatigue cost
- Strength holding or climbing between relapses
- Fewer falls and near misses
- A flexible program that adapts to the condition instead of fighting it
