Support at Home

Ageing at home, with strength to enjoy it.

Support at Home funds allied health so you can stay independent where you want to be. I deliver 1:1 exercise physiology for participants across inner-south Canberra, through your registered provider, at my Griffith clinic or in your own home.

What the sessions are for.

The evidence is simple: strength and balance training is the single most effective thing older adults can do to prevent falls, stay mobile and stay home. Under Support at Home, that training can be part of your funded care plan, delivered by a university-qualified Accredited Exercise Physiologist, not a generic support worker with a resistance band.

Programs are built around what independence means to you: the front steps, the shower recess, the shops, the garden. We measure where you start, train what matters, and re-measure so you and your family can see the difference.

How to set it up.

  • If you have a Support at Home budget: tell your care manager you'd like exercise physiology and pass on my details, 0418 153 608 / admin@fwphysiology.com. I handle the arrangement with your provider from there.
  • If you're a provider or care manager: I'm an ESSA-accredited EP taking Support at Home referrals across inner-south Canberra, call or email and I'll send my service details and availability.
  • Not sure what funding you have? Call me and we'll work it out together, and if Support at Home isn't the right door, a GP Chronic Disease Management plan often is.

How it comes together.

  • 1. Ask your care manager to include allied health, exercise physiology, in your care plan, and give them my details
  • 2. Your provider contacts me and we set up a simple service agreement, no paperwork lands on you
  • 3. Sessions start, at my Griffith clinic or in your home, at a time that suits your week
  • 4. Your provider is invoiced directly from your Support at Home budget, nothing out of your pocket

Home or clinic, your choice. Some clients prefer their own hallway and kettle; some love the routine of coming to the quiet gym. Both work. The program comes to wherever you are.

How do I access exercise physiology under Support at Home?

At my quiet Griffith gym, or in your home, whichever suits your mobility and goals.
