How it works

Medicare CDM, the basics.

A Medicare Chronic Disease Management (CDM) plan is written by your GP when you have a chronic condition expected to last 6 months or more. As part of a Team Care Arrangement (TCA), it entitles you to up to 5 Medicare-rebated allied health sessions per calendar year.

Exercise Physiology is one of the eligible allied health services under CDM. Your $63.40 Medicare rebate is claimed on the spot each session, you pay on the day and the rebate lands straight back in your account.

How to get a CDM referral: Tell your GP you'd like to see an Exercise Physiologist. They write a CDM plan and a Team Care Arrangement. You bring this to your first appointment at Fitness & Wellbeing Physiology, or your GP can fax/email it directly.

Conditions that commonly qualify

  • Type 2 diabetes and pre-diabetes
  • Cardiovascular disease, post-MI, post-stent, heart failure
  • Chronic musculoskeletal conditions, osteoarthritis, chronic LBP
  • Mental health conditions, depression, anxiety (with appropriate GP referral)
  • Obesity and metabolic syndrome
  • Hypertension
  • Neurological conditions, MS, Parkinson's, post-stroke

A Medicare care plan gives you five allied health sessions a year. Most people never use them, and many who do get five disconnected appointments that lead nowhere. I treat those five sessions as a project: a clear assessment, a plan aimed at the goals on your GP's care plan, and measurable progress reported back to your doctor after the first and final session, because that is both what Medicare requires and what good care looks like.

The money is simple and published: each session is $150, GST free, with your $63.40 Medicare rebate processed on the spot, leaving $86.60 out of pocket. No surprise gap, no pressure to buy more than the plan provides.
